Covid-19 Vaccine Campaign for 5 to 11 Year-Olds

Families have two options.

Option 1

Make an appointment and bring your child to a local vaccination centre through ClicSanté

Option 2

Have your child vaccinated through school-organized vaccination clinics.

Depending on location, schools will be organizing either in-school vaccine clinics, or “field trips” to local vaccine centres. Schedules, documents and links are included under each school name.

Download, print, complete, sign, and return the following two documents to the school by December 7.  (Contact CPS if you need hard copies of the documents.)
